飛行機を乗り継ぎ、カーターはアフリカの奥地、キサンガニを訪れる。そこで待ち受けていたのは、医師や看護師はおろか、医療器具や医薬品など、すべてが不足している劣悪な環境だった。その後マテンダでコバッチュと合流、懸命に医療活動を続けるカーターたちだったが、突然、武装した民兵たちに取り囲まれるという絶体絶命の危機を迎える。
